Medical Forms
Every student participating in programs at High Trails is required to fill out the High Trails Medical Form. School staff play a major role in distributing, collecting, organizing and communicating with families about medical forms and protocols. High Trails nursing staff needs the complete school set of forms at least one week before arrival. All pages of the 4-page form require parent and/or doctor signatures. Please check each page for signatures prior to your High Trails visit to ensure completion of the medical forms.
All students are required to complete:
High Trails Medical Form Page 1 and Page 2
Any student bringing any over-the-counter or prescription medication including inhalers, epi-pens, herbal remedies, vitamins, etc... are required to complete:
High Trails Medical Form Page 3 (parent signature) and
High Trails Medical Form Page 4 (doctor signature)
